The balls are dropped straight into a bottle of paint and move freely in it when it is shaken: they stir up the contents, break up the deposit at the bottom and mix the pigment back into the binder. You will never mix a bottle that thoroughly by hand alone.
The material of the balls is what decides here. Glass is chemically inert: it reacts neither with the paint nor rusts. Metal balls can gradually corrode in paint (above all in water-thinnable acrylics) and rusty particles then ruin the paint irreversibly or discolor it. A glass ball left for months in a bottle will still be a glass ball. That is why glass is considered the safe choice with modeling paints.
